Work with your counselor/coach and supervisor to confirm your understanding of what they didn’t like and agree to a realistic plan with milestones to demonstrate your progress to remediate and grow from it. Do this even if you’re not forced to as it will help build credibility with your project team and business unit.
In annual reviews, a person who made a mistake and demonstrated an ability to bounce back is often as impressive as someone who just had an unblemished year.

Be intentional everyday about your performance. Ask for frequent feedback. We are at the beginning of the performance year. You can totally turn this around.

Sometimes you can start a project without a success statement at the start, but that is risky. I agree you need to talk to your CC and find a way to quantify what you need to achieve. Then check in once a month for feedback.your role needs to first show contribution to scope, then include what you want out of it.
