Keeping your manager informed is never a bad idea. Now leadership absolutely needs to know. I assume he rolled off and wasn't laid-off.

That is why I said inform first. I'd suggest next time you work out a plan on day one with the person rolling off, and send an email documenting it to your Manager. Then ping and send emails when things slip. On the 4th day or so, escalate.
