By certifying your claim each week you effectively keep it open. Just say you worked 5 days and earned money. But when they ask if you’ve “returned to work” say no. Because you haven’t taken a full time job technically. When your W2 gig ends and you certify that week, give the reason that the work “ran out”. Don’t say you quit or got laid off. I am in NY but I’m making the assumption that the two systems are similar.

For the W2 they will know where you worked as the agency will submit your SSN to them when you fill out your paperwork. So you should say the address if asked. In NY they only ask for the employer once the W2 ends and you need them to start paying benefits again. That’s why you have to say the work ran out. If you say you quit then you won’t be eligible. It’s very technical!
