You’re too close in levels for this. If the m/sm/ppmd doesn’t find it worth their time, then I believe the kids these days say “stay in yo lane.” It’ll come across poorly on you.
And heaven help any struggling analyst working under OP when they make manager. I remember having a hard time when I started out. If someone was gunning for me like this rather than trying to push me forward, I wouldn't have progressed like I did over the years.
If you didn’t being this up during the project, then you lost your chance to being it up at all. I disagree that it’s not your place to say something, as sometimes Ms and above are not aware of what’s happening in the trenches. However, dropping a grenade on someone after the fact, without giving them a chance to rectify is cowardly and looks terrible for you...

Honestly it sounds like OP has a personal issue or grudge against the analyst. Clearly it isn’t anything work related.
You don’t really have the experience to make these assertions. If the manager did not reach out that means that they made the judgment call that it was not necessary.
How do you know they haven’t said something. If I was a manger telling a CC about a low performer I would tell the low performer and then if necessary, their CC. But not everybody else on the project, especially those not above me.
Don’t do it, people get let go for completely random reasons at Accenture instead of potentially legitimate ones - I’ve seen people rise up to SM that should have been let go early in their career. It is NOT a meritocracy so get over that notion immediately if you still have it. It’s like a casino it’s about playing the games and chance
A1- yes. I’ve never met anyone as terrible as this person in my entire life. It got to the point where I almost had to roll myself off the project because this person was toxic- both professionally and personally, though all documentation provided to leadership was strictly around work related matters (i.e. blowing off client deliverables, not working/even being reachable during work hours, etc)
Did you talk to the manager about this during the project? If so, what was their feedback/thoughts?

Agree with SM. What's your desired intention here? What's your desired result? I think you're broadly saying you want to be a good guy and warn analyst. But think specifically what you want to happen out of that interaction/dialogue
SM is right. If you didn’t have conversations with the M during the project and especially with the A then that’s unfair. If you did have the conversations, request the A to send you a Snapshot (make sure the M knows you’re requesting it) then rate accordingly. The CC will be informed through the ratings.
I definitely don’t think it’s your place to reach out. If you really cared, you should’ve offered feedback to help them as a peer and a professional. If you thought it was so toxic but never told them in person, I don’t know how you expected things to improve. They got rolled off - let them live and let go of the spite.
P2 it honestly sounds like self preservation, or just flexing a bit to emphasize that they oversaw the person. Honestly, everything was handled, and I would bet that the CC is aware of what happened. Best to leave it as is to avoid seeming petty.
I have tons of documentation of this Analyst's “actions” (or really lack thereof). This person should not only NOT be promoted, but really needs to be on a PIP...

A3 - kind of agree. Why reach out to their CC after they're gone rather than project leadership while they were there? Is the goal to get them fired? Ensure they don't get promoted? If they've been rolled off, you should be fine now. And honestly, the CC probably won't do anything about it. CCs generally advocate for their counselees whether or not they deserve it unless their documented feedback is just awful.
