I took one and it was life-changing. Was so recharged and in a much better mental and physical state after returning. That being said, mine was a firm-paid milestone promotion award.
Just want to throw this out there. This type of stuation may qualify you for medical leave. Look into that first and ask for unpaid leave/sabbatical time to extend if necessary.
Take care of your mental health the same way you would your physical health. If you do take time off, make sure you take care of as many things are on your mind so you do come back refreshed. For example, if you need a month to add a few sessions, you may want to fit in a mini vacation or use that time to check off home improvement work, or get back into cooking. You just don't want to take a lot of time off and then come back feeling like you need to take vacation time for other things.

I did but as a more junior employee after baby - came back to a travel project and just wasn’t feeling it. I basically said I was doing it for baby bonding etc. it was fine, but people are definitely surprised that I came back rather than just using that time for find a local less stressful job.
So for my firm, it automatically uses up all my PTO etc before going unpaid - when I stopped accruing benefits. We can stay on the health insurance, but would have to pay for it once we were on unpaid leave. I was not on the insurance etc. I don’t quite remember for sure what happens to the life insurance and disability insurance etc but think those stopped... basically because they stopped paying me and I didn’t owe them money for health insurance, I could’ve just not gone back and it would’ve counted as I quit. You have to file paper work and notify them of return, I think - which I did in conjunton with a project starting that I was joining.
