Coverage of something like this (which is generally considered a luxury) is dependent more on your specific plan than your insurance provider. Very few insurance plans will cover it unless there is a medical reason for it, such as you’re about to undergo chemo (which would make it medically relevant as opposed to a luxury/voluntary procedure).
As far as I understand, it’s kind of covered the way plastic surgery is, where if it’s voluntary it wouldn’t be covered unless it is deemed medically necessary/relevant, such as reconstructive surgery.
Some employers do cover it themselves, though it is rare. Sometimes found in the tech industry if you’re working for a large company (ie Microsoft covers it)
