Unfortunately it is very common in smaller companies that as people get promoted to leader roles they do not receive the proper training and mentoring.
In some scientific places, post-docs will start as group leaders without every being individual contributor, which could easily end up creating and exacerbating a toxic workplace, because they don't know any better than what it was like in academia.
In most enterprise companies, as soon as someone gets promoted into a leadership role, or even if they're an IC getting an intern, they'll immediately get to follow a set track of trainings. Most of which are actually about communication (giving feedback, meeting efficiency, etc.)
The above is based on my own experience and YMMV. I've experienced bad senior managers who jumped from startup to startup and simply never had the proper training or possibility to gain the proper experience, which causes them to become severely biased but also have very low self esteem as they realize they're out of their debt. (To whomever reading this, if you recognize yourself in this, listen to the experts on your team, and ask your manager for the proper training and coaching to improve your people leader skills)
