I work in healthcare accounting, so we deal with a lot of PHI. A client sent in support by email with PHI (not allowed), so I was supposed to report that security incident to a manager. I was working with another accountant (she’s from another office), and I just assumed she took care of that. I just deleted the email and moved on. Hopefully they never find out. Besides, this was over 1.5 years ago.

Leading a conference call on speaker phone at client site for a status update with the SM and partner. client innocently comes into the room to say hi to me only to hear the partner on the line smack talking the client’s competencies ... yeah to this day the partner did not know the client heard it and that incident never got brought up by the client like it never happened ..at least to my knowledge
