I tried diet delivery subscriptions, it really helped. You get delivered tailored menus that fit your goals and when you feel like you can do it yourself you can switch it off. When it comes to exercise, I try to only practice sports that I actually enjoy otherwise I get bored and never come back again

Don’t set unrealistic goals - that’s an easy way to get “off track”.
Build a consistent routine - even if it’s a 20 min walk that increases over time, of lifting three days a week that grows to five, heavier weight, more reps. The routine is important.
Don’t think of it as punishment - see it as an investment in you.

I would try to identify what’s causing it. Is it the busy schedule or lack of motivation or something else altogether. Then from there, start with small steps. May be schedule 2X a week with a personal trainer, join an accountability group, etc.
You need to get into a consistent groove. Doing both at the same time is mentally tough but the most efficient. Force yourself to get up early if you’re working late (take preworkout if lifting), treat protein shakes and green smoothies as your reward, and over time just try to rewire your brain
