INCOTERMS don’t address title transfer. The delivery point set out in the selected INCOTERM, can be used as the title transfer point, but that would need to be separately spelled out. UCC 2 does address title transfer to an extend, assuming it’s a domestic transaction and governing law is well identified. Still, I always try to specify a distinct point in time where title transfers.
As to whether buyer having title “at all times” is standard, I would say no. I guess it ultimately depends on what’s being sold, how hard it is to replace, and what the payment structure is like.

Yep i would unbundle title into all the things encompassed in title - risk of loss, literal ownership, who bears cost for shipment. Incoterms addresses the key business points of what is captured in “title” - risk of loss and cost of shipment - and secondary sources will give you a roadmap to what is market.
Not sure I fully understand your fact pattern, but rather treating this as passage of title prior to delivery, I sometimes see provisions for the buyer to purchase “work in progress” in the event of termination prior to delivery.
