It's easy even if you don't have documentation
You can do it online with nothing but your medical history and an explanation of the problem to a doctor. If you already have a primary physician I would just discuss it with them and they will probably write you a prescription. Just started again after not taking any since before middle school, I'm 29 and it has been crazy to see the change in my behavior.
You'll likely have to be diagnosed again, or at least have your prior diagnosis confirmed by your new doctor. Shouldn't be too hard, though. My diagnosis with no documentation was relatively straightforward, and I had no issues getting on meds. Kudos to you for admitting that you need help and doing what you need to do to get that help.
I'd rather not bring my medical history records because you will get likely to be diagnosed with a recurring issue. But i think, you can still manage with conventional remedies. Have you tried doing yoga? It helped me maintain the normal function of my brain.
Going to a physician and getting rediagnosed would be your best bet. You'll get to see if it really is just a recurring issue or something other.
