I use headphones all the time as the incharge of a large job. I don't think the partner is a fan but then again, it helps me get shit done
I would be more aware / cautious at a client depending on their culture. In the office I personally could care less. I fee like 10 years ago this was a bigger issue but everyone has adapted now. The only other thing I would caution is if you are sitting in a room with more senior members of the team I always found it interesting or helpful to listen in to what was going on. I felt like it made me more aware.
Generally no, but I put one headphone in (facing the direction away from the team) if I’m with a new team at first. Or get some wireless buds and grow your hair longer and nobody will know 😉.
At the office I don’t mind in any way. At the client, I’ll talk to the client first to ensure I don’t get a sense that it’s counter to their own culture. I’m personally more focused when listening to music.

Etymotic headphones have been a life saver!
I think headphones in the office are just fine. I bring a Bluetooth speaker with me when I’m in the field. I usually let the staff pick the playlist (within reason of course).
As much as I would love to do 2 headphones, one is usually what is accepted. Had managers who would get pissed if you couldn't hear them if they started to rattle a task off to you. But occasionally would go for 2 (to try and crank out something) and would tell someone close by me to nudge me if they were talking to me
