We did have to send in the hospital's "kid was born" form to activate our baby-bonding leave. Google outsources handling of baby leave to a firm called Sedgwick, who asked for the proof-of-birth form. Fairly easy: emailed in a photo. We then each (my spouse is a Googler, too) got 3 months to use in one-week blocks, which all expire on the kid's first birthday. As the one who gave birth, my wife got additional pregancy/disability leave, in a separate but similarly low-stress process.

Oracle used to have a 1 year time frame. You can take parental leave 10weeks at anytime within the first year of your baby's arrival. 10weeks all at once or broken out in x+y+z =10weeks throughout the first year. Not sure how the benefits team checked on the 1 year. Your manager probably knows because most people are out the first week a baby arrives.
I believe the limit is within the first year. My husband took 4 weeks then took 2 weeks right before our child’s 1st birthday. It is a headache though i would not recommend it in blocks. You hsve to deal with HR, State disability (at least in CA - your state may vary) and any short term insurance claims.
