I generally do though didn’t last year. I ask parent their names since last names vary; how they’d like to be contacted or if I use DoJo let them know; strengths and interests of their child and the personal goals they have for their child. I share Resource Room/Learning Center expectations and after school study opportunities. It’s been helpful. Plus it’s a positive initial contact. I’ve distributed questionnaire a couple of ways:
1. Mail with stamped return envelope
2. Have their classroom teachers distribute during their personal time during our open house.
Best results come evening of open house. Mail out the rest.
Constantly tweaking to make mutual communication better.
I don’t usually, but I will this year. This will allow the parents to feel a little relief knowing that we are aware of their child’s strengths and needs.
I am curious to see what others say. I would like a sample too if possible.

Depending on age... I teach ECE/pre-k and many of my students have speech delays. One of the things I have asked is also names of pets and siblings, along with what the child calls everyone (esp grandparents, such as nana). It makes the guessing game much easier when talking with students.
