I have also left a job after 3 months for the exact same scenario. When asked I explained that the facility was not a good fit for me due to the lack of autonomy with regards to patient care and staff.
I think being honest but tactful is the way to go. It wasn’t a good fit for you and you didn’t feel patient safety was a priority so you felt best to terminate employment at that time.
I was working for a company when I first graduated and was only there for about 3 months and it is on my resume for the reason when I interviewed for the company I work at I was transparent and explained the situation. I was called back 3 days later and offered the job I believe that my transparency about the position was the reason I was hired

Whats the vibe you get from this place? What does the prospective employer say their values or mission is?
You could be forthcoming and say it how it is. Or you could describe it as a “bad fit” or “didn’t follow my core values as a provider”
Omit it from your CV/resume, I doubt you were fully credentialed in that period of time.
I see, so then just be honest, It didn’t work out because it was toxic, provide a brief explanation and thats it.
Tell them exactly why. The other place was awful. That's not your fault.

Say the position wasn’t as advertised and you wish they find the perfect fit as well as yourself
You could just say the office wasn’t a good fit for you. That you are looking for ——-(fill in the blank) and hope to find it at this location/position.
Credentialing Manager here. You’ll be fine. Put it in there so there’s no gaps. Just explain the situation. :)
Leave it out of your resume. It’s better not to say anything negative and they probably won’t even notice a 3 week gap.
