Does anyone know how people who already filed their AOS (i-485) will be affected by this? I'm vaccinated but my AOS was filed in March along with medical examination and I'm concerned this will delay the process even more :(

Effective date 10/01, can't get anymore clear
“This requirement is effective Oct. 1, 2021, and applies prospectively to all Forms I-693 signed by the civil surgeons on or after that date”
This means if you have submitted 485 and 693 prior to 10/1, you are good. But on or after, vaccination proof is must.

If you’ve already provided your 693, worst they can do is send you a medical RFE and you send them an updated 693 with proof of vaccination, shouldn’t be a problem.
