Are you sure that would all be covered by your medical insurance? FWIW, my wisdom teeth surgery was covered by my dental insurance, not medical. I’d also check on the coverage for vision therapy. May be stating the obvious, but I’d definitely keep that in mind when weighing your options!
A big component of how much your wisdom teeth will cost is the anesthesia. I was under general anesthesia with an oral surgeon. If I had to pay out of pocket, it would’ve been thousands - exceeding either of those deductibles. If you are comfortable with local anesthesia or have simple extractions where they just need to yank them out, it’ll be much less expensive.
Wisdom tooth extraction is covered by dental, not medical. Vision insurance is usually separate as well
Yes they are separate. Vision therapy will be under medical bot vision though. My vision insurance is just for lenses and eye exam.

Probably the more premium plan. Although you didn't state out of pocket maximums, the general idea is that HDHPs are good to save on premiums if you consume little/no healthcare services. I assume that means you'll get screwed over with coinsurance or OOP max if you do use a lot of healthcare services.

Go with the PPO. The premium cost savings for the HDHP are not worth the risk that the care you need will cost more than expected, and the premium savings aren't worth it.
