I’ve always done EPO/PPO because that way I don’t have to think twice before visiting the doctor and my annual healthcare costs are somewhat predictable.
Plus it allows me to get $15 copay massages and acupuncture on a regular basis.

If your employer helps with an HSA and you don’t have major health issue, uhdp is the better answer generally
I do HDHP, one month of prep basically knocks out the deductible (I ultimately don't pay a cent for it).

Director1 is referring to a trick that I also used to do, which was to have a high deductible plan, get a prep prescription, filling it would cost thousands of dollars, then Gilead would cover the uncovered amount as secondary insurance (more simply described as a coupon card)....I'd be out of pocket $0 but cleared my deductible at the same time.
However -
The latest health care rules include Prep as a preventative care item which all plans must cover without any deductible.
This means that filling your prep prescription can no longer be a way to cheat your way out of a deductible. Your prescription will be free in the first round -- you won't require the Gilead card and it also won't give you any credit toward the deductible for the year.
HDHP lets you sock away thousands in your HSA. Invest it and it grows tax free. Then just pay near term medical costs out of pocket.
I think there are a lot of other circumstances to think about other then STI testing and prep. That’s very planned which is fine when doing the calculations, it’s the unexpected that you should think about. My PPO plan is amazing though my company. My total bill for getting cancer last year to insurance was $156,000 and I paid $832 out of pocket at end of the day. My medication with my PPO is $75 for a 90 day supply or $1585 for 30 day supply on HDHP. You can’t just look at the little routine things when making the decision in my mind but I know it works out for some people
