Tumi backpacks are probably your best bet. And they have the little sleeve so you can slide them on top of carry ons.
Just buy a bigger tote with a strong base, and keep it on top of your carry on while you're on the road. If you're pregnant, you shouldn't have to worry about carrying something on your shoulder or your back. Good luck.

I was having back problems and bought a black lulu lemon backpack. Honestly, it fits way more stuff than any of the tumi backpacks I saw, less expensive, and pretty cute.
I'm on a local project now so no carry-on to roll around unfortunately. Agree that laptop case on wheels is probably the best option but overall those ones weigh a lot so I'm a bit hesitant (carrying it up and down the stairs to the subway). It's possible that backpack will not be much better on my spine but at least it's better weight distribution on both shoulders rather than one.

I have the tTumi Olivia convertible backpack
This from a male si forgive the ignorance. Just thinking about the load disbursement on the spinal cord with the baby in the front and back pack in the back. Would you not be better off with a laptop bag 💼 with wheels that sits on top of the suitcase when walking at the airport. Lastly with obvious pregnancy size enhancements do you want a backpack 🎒 to pull on your top?
