I know of colleagues who have used FMLA - I personally needed to take two weeks off last year when a medication change really pushed me into uncomfortable levels of anxiety/depression. I used PTO for near term and over the course of those two weeks my medication change rebalanced and I made the decision to come back but we were exploring short term disability and other options too. If you feel comfortable talking to your manager about it, you can go that route (which is what I did) - if not, I would look into if your organization has an Employee Assistance Program where they will be able to answer any and all questions / provide options (and provide more anonymity). Give yourself some space and time to work through things - you got this!
My therapist doesn’t do FMLA/disability forms for objectivity reasons, and a psychiatrist I saw wanted me to try medication first — but I feel pretty strongly against taking an antidepressant (months-long commitment) because of a job
I’ll probably just do regular leave then...
