Traveled back from India to U.S. via Bahrain and Heathrow through two different airlines on Friday. Nobody asked for any test or vaccination proof across 4 airports - Chennai, Bahrain, Heathrow, and JFK. But, I did carry all the vaccination proofs with me. I read up and found that test was not required across my itinerary.
Both required but saw a news few days ago that they are thinking of changing it.

Travelled to US from India last month (July) did not require PCR test. I had my vaccine card with me, but it was not checked either
Ind->US is not required.

Us to India- you either need rt-pcr test or vaccine
