I find that time boxing really works for me!
Time boxing has been key for me as well. Not on medication yet but for whatever reason I have such an easier time addressing specific tasks if I intentionally set aside time for them as opposed to just having meeting / open slots on my calendar

Try Implementing urgency and novelty into your work.
For novelty maybe restart a task from scratch with new eyes, use a different approach, or go work at another location
For urgency, ask someone to keep you accountable for meeting mini deadlines. Set timers. Write a list of 2 things that you absolutely must finish today. Recognize the accomplishment of finishing the two things and take a physical break before tackling 2 more things.
First, I’ve been exactly where you are. I could have written your original post earlier this year. I can tell you what I’ve done to get from that spot to being still less efficient but at least moving forward and getting good feedback from my colleagues.
I agree about time boxing, especially time boxing the entire work day first thing in the morning. Link doing that to something you do every morning to make it part of your routine. I do mine in Outlook and make “appointments” where I’m either busy or available depending on the urgency of what I need to do in that block. Color coding the blocks helps me see how many billable hours I’m clocking and really helps when timesheets are due.
But then other people start working and want to message me about what they have decided is an important use of my time, so then it comes down to negotiation skills and making adjustments. These both take practice and I still find myself overcommitting too much of the time. When things get crazy, make myself unavailable in teams with a message that I’m focusing on a task and will respond by a specific time. I use that option sparingly and so far so good.
Your need is urgent now and I hope you get more good recommendations here, but for the longer term maybe look into finding a coach who specializes in helping folks like us. Pills don’t teach skills, as they say!
