I don't have a template, but I would include metrics around total pipeline, deals closed among all sales team, most common closed-lost reason, maybe a comparison to other companies in the industry if possible (to demonstrate industry-wide conditions if applicable.) It sounds like you think your poor performance was due to personal issues, though? Might be a bit challenging. Have you spoken to manager or HR about those issues? Might be best to get a bit more understanding and honesty around whats going on.
I would recommend you go to HR about your situation. There are programs in place to assist with loss and any down moments you’re experiencing.
