If you know the above to be true then I'd go to HR with your story now and ask how they recommend you proceed. Make sure your partner leaders know as well. If you didn't do anything you should be safe. Document all though!
OP, are you kidding when you speak of chances? You already gave her a few. This is a race to HR. Whoever gets there first, makes a better, untainted case. Why are you hesitating? The innocent have nothing to hide
If you did not do anything wrong I'd bring this up to HR that she is threatening you.
If she doesn't care about her job and hates PA what's the point of trying to coach and keep her. No firm wants these types of employees. Except maybe the Dallas Cowboys. They like having criminals.
Get to HR before she does. Make sure your counselor and engagement partners know. Document everything.

Threatening you in this way is blackmail and extortion and a felony. Tell HR and consider talking to the police.

I would go to the partner on your team ASAP and ask how to handle. Appreciate you want to give a second chance but it sounds like they have had a lot of chances. If you are highly rated, have no history of abusive treatment towards others in your firm, and can have others back up what you say, I would think you should be fine. However I would get in front of it ASAP. This person sounds toxic and should be let go.

Hey everyone! Sorry for the delay, just wanted confirmation before I posted. HR has dropped the complaint and the individual is leaving the firm. Thanks for all the great advice!
5. She openly admitted to other staff that she doesn't want to work in PA and doesn't care if she gets fired or quits without a job, but will not leave until she gets me fired
6. None of the allegations are true. She is claiming I did not provide coaching, that I called a controller a c***, etc.
Side note: I've been a high performer since I started and have been recognized for my coaching ability and client relationship building above all else. Not to mention, if anyone called a client a c***, the partner would certainly hear of it
How does someone threaten you out of nowhere. There maybe an atom of truth in her story. Why you? There are lots of seniors and managers she would probably have worked with but she chose you
