No transit visa needed.
Take into account the time needed to change terminals (if you need to change terminals) and try to hurry through the process of exiting flight 1 and checking into flight 2. For my journey on the same route few years ago, we had to take a short trip on a bus to change terminals, which definitely added to the minutes.
Check the terminal information, should be available on airline or Heathrow website.
1 hour could be tough - gates usually close 25 minutes prior to departure.
I never choose flight options with less than 2 hours gap irrespective of airport layout. United could put you on another flight but you will lose time (next flight could be 4-6+ hours later)

Yes you do!! My friend was denied boarding 5 months ago. Assuming you have an Indian passport without a valid U.S. stamp. Check rules in case they’ve changed.
You really don't need any transit visa, it's really random, sometimes they check visa while boarding flight, sometimes they don't, when my wife traveled they did not bother checking anything and her visa was expired, but when I traveled they checked my I797 to make sure it's valid as I too had expired visa...it all depends on who is at the check in counter that day and what mood they are in!

It will happen at the US airport itself when trying to board London flight. If you make it to London without anyone questioning you at US airport, it should be ok from there for India. I am not 100% sure but it seems it has something to do with deportation. So assume you went to London and for some reason which may never happen you need to be sent back. Now since your US visa is expired they cannot send you back to US. Hence the rule for valid visa.
