What is the special allowance part in the CTC breakdown of Cognizant? It is nearly same as my Basic Pay, Just wanted to confirm if that amount is the same for other people also? For any kind of planned leave always inform over email - 15 working days prior to your manager.
Approach should be like -" I will not available for these following days For some personal work . would like to apply for PTO for the following days . "
Note - Email should be like - you will not be available & informing to manager - it should not be like your asking for approval & then only you will take leave .
I always followed this tricks & never get any obstruction in my 6 years of tenure .
Say it should be like informing not like asking/requesting. Let's see what will happen. But i learned a good lesson.

Always remember my friend, do not intimate such things on call or chat,
Once discussed, drop a 1-1 mail to manager so at last when manager deny you will be in safe zone.

I knew this will happen, so had intimated it by mail. And it's not like others had not taken long leaves b4 me.
If they deny I'm ready to escalate it.

Cognizant has the policy of auto approval of leaves if it's not approved by the manager, if manager rejects they need to provide a proper justification.
Since you already applied for the leaves 3 months ago, I'm pretty sure it would have been approved already, the manager cannot go back on his/her word now.
You can always include HR in these discussions and inform them of these issues, if it goes unnoticed, please take it to your management. They should not be playing with your ratings for their weak egos if you do take the leave anyway.

In Cognizant HR is visible until you join the company. Once you join they miraculously disappear. Even a "Ghor Tapas" will not help you but once you use the "Brahmastra" of resignation they suddenly appear. And not one but many HRs become visible at the same time.
