Soil can be exhausted if planted in too many times without amending. It can also get too dense over time but that's only in soil mixtures. Soilless mixtures like cactus soil take forever to break down. Is that what you mean?

It may just be picking up some residual humidity. If you're concerned, you can always sterilize the soil on a cookie sheet in the oven. That's what I do if I bring in outside compost for my inside plants.
Soid doesn't go bad, its power just diminishes over time. Refresh the soil when repotting every 1-2 yrs and you shouldn't have a problem. You don't need to strip the old soil from the plant just put fresh soil in the new pot, lightly shake off excess soil from the plant and repot.
However, if you have visible gnats or bug flying about the bag or if there's a rotten odour coming from a new bag you shouldn't use that. Return it and get a different fresher bag.
