For clear-cut promotions, the manager has to submit the request with the documentation to support it. For internal transfers, the employee can submit an application themselves to prove they are qualified for a lateral move or "promotion". I'm not sure if this is typical, but it's worked for us for years.
I’ve been at different companies with different practices. One had an annual promotion period at the end of the year. Another did it twice a year. My current company is not bound by a certain period of time. All of them were based on a mixture of time in role, substantial changes in scope or responsibilities, span of control, # of direct reports, revenue of supported client groups, etc. People were promoted within role based on these criteria, or applied to open positions.
Theres an semi annual open period for promotions. Comp or HR publishes an announcement for open period. manages submit a form through HRIS- following the guidelines around promotion eligiblity such as length of time in current role etc

We are a smallish non-profit (110 employees). If a position opens up, an internal candidate needs to apply to express interest. Very rarely do we approach an employee to offer a promotion, maybe unless they have been specifically developed to grow into that position.
Usually after a year you can be considered. They don't like to ocnsider anyone under 18 moths tho really.
