Depends on how many documents you review. I bill for review and drafting of an outline. But some clients don't let you bill for drafting an outline. Do you have lots of medical records to review? That can take a while

Bill for reviewing each document, unless the documents can be grouped together (e.g., medical history), and if so, then write the page number of the grouped documents (e.g., medical history of John Doe, 25 pages, re: preparation for disposition of Billy Joe).
The total amount of hours to prepare, say 5 or even 8, shouldn’t matter so long as you can support it with the documentation you reviewed.
