I had PD on it and found a free transition curriculum on Schoology. Recently, I have been teaching 30 minutes a week on Casels SEL competencies. I tie it into transition. It a great way to show how social emotional learning is important in the real world. I also use Nearpod lessons for transition. Not only do they have SEL but also post secondary and employment interactive.

I believe you become more knowledgeable through professional development, self-discovery, and by actively communicating with outside agencies that support individuals after they transition from the high school setting. It is helpful to be familiar with agencies in your area that serve individuals with disabilities as well. There are also free checklists that educators can use to assist them in writing goals and providing services to their students.
My school district hosts a transition fair every year. Invitations are extended to every special ed student and their families. They encourage participation from 6th grade on.
I try to attend a Transition Conference each summer. I also try to take my students to the Transition Fair each year. Attending both of these really helps with networking. We also have Pre-Employment Transition Services (PETS) meet with our students once a month. Voc-Rehab and Preferred Family Health also meet with my current juniors regarding the Summer Work Experience Program (SWEP). Getting the students involved with them early helps get their files opened early with Voc-Rehab their senior year. I've also attended TOPs training.
My covid brain can't remember exactly what the acronym stands for, but the training is great! They teach you how to use the transition plan, Form C, to drive the rest of the IEP and meet all the standards and indicators.
