Maybe download a blank one from somewhere to use as a guide. That way you have a structure.
You could ask hr for a copy of your job description, or see if they’re advertising the role now online and grab the job listing. For each component, list your achievements/contributions. Ideally you can illustrate how you’re exceeding, not simply meeting, the criteria.
For future. It helps to make a designated appraisal folder in your outlook/gmail. As you receive positive feedback throughout the year, drop those emails into that folder. If the positive feedback was spoken, write it up in an email to yourself and then save it to the folder.
Come time for reviews, you’ve got a treasure chest of positive feedback that you can directly quote in your write ups!
Earlier in my career, I remember seeing my boss at the time, whom I greatly admired, scribble down positive words from a client whilst she was literally receiving that feedback in that client meeting, without even breaking eye contact with the client. It got my thinking... at every single level we need to be experts at recording memos of positive feedback. There’s no shame in that!
Usually when it’s a more structured form, they ask you about your accomplishments, your goals, what you want to improve on (note this could be acknowledging a weakness but also could be something you want to learn more about or get more experience in)
