I doubt it’s HR. More likely someone knew someone at your former firm.

I did and asked for feedback and details but got completely ghosted which is what i expected since im fairly confident the reason they didnt move me forward was due to the disability so of course she couldnt say anything remotely related.
It’s really messed up if it’s a disability. Unfortunately there’s not a lot that you can do that will result in a good outcome. But on the bright side if they are that kind of company up front maybe it’s for the best in the end. Would they have been accommodating to you as an employee?
Accenture only validates employment term, position and last salary during employment verification. They don't give any other detail. It is not even HR that validates it's a third party company Accenture hires.
So for salary you actually have to authorize it.
Affected my candidacy. I know this because they asked me questions about it during my interview and there’s no one at the new firm that has worked at my previous company. Part of me feels like this is a common practice possibly hr having friends in hr at other companies. there’s absolutely no way they would’ve known about this otherwise. 
So what're you gonna do about it
If they asked you about your disability that’s illegal and your rights are federally protected by the 1973 Americans with Disabilities Act. Now in terms of proving it, I’m not an employment lawyer so could be hard to prove or not. But either what that’s completely inappropriate and illegal.
I’ve heard of similar things happening when someone moves from Accenture elsewhere. It sucks but if they have the balls to ask you about something which is federally protected, you probably don’t want to work for such a company. I’m sorry this happened to you and I hope you find a company that’s the right fit.
My firm asks because “We do business with the government” it’s optional to fill out, but it helps a metric somewhere I think.
Sounds like they uncovered something pretty radioactive. If it could hit your candidacy with mere knowledge of it then it’s a good thing they did. HR is trying to protect the firm and the employees at that firm, that’s their job.

You must have done some bad shit at your previous employer so it is the responsibility of HR to share when they call for employment verification. It’s to do with disability then it’s completely different. There are a whole bunch of rights and laws to Protect you.
