I could not understand 20% of the interview questions due to accent issues and answered them as per best understanding.
I think I might get a rejection.
Would it be a good to notify HR about what happned or should i wait for result?

Agree w using this as motivation to make life count now! We have ONE life to live and better live it as well as we can. No time to waste.
That being said, part of living a great life is not beating up on yourself or hiding yourself from others because you’re unhappy w yourself or embarrassed or depressed. Try to turn this over in your mind also. Go out there and engage w those you like and miss! And don’t be so hard on yourself. I bet if you did an inventory of all the accomplishments you e also had in that time there would be many on that list too. Don’t underestimate how awesome you are and how far you’ve come too

got fixed. I thought I worked so hard, but I consistently gained weight, my pictures got worse and worse each year, and I became less and less social and more and more run down...until now I barely even have a picture a year to scrape together because no one has been taking my pictures!
This makes me want to take massive action to fix my deal. I mean how did this go so wrong? I was working out SO much! Why didn’t I lose weight? Where did all my friends go? Did that one relationship break up at 22 ruin the rest of my trajectory?

It is never too late to love who you are. If you want to get in physical shape 38 is nothing. My dad was overweight and in bad shape most of his life post kids... he had s health scare in his early 60s, had to go to physical therapy, and his physical therapist made him sign up for the gym and go (apparently he asked what my dad wanted to do in 10 to 15 years and he said "play with my grandkids", PT told him unless he thought toddlers would be into playstation if he wanted to do that his fatass needed to get serious about moving his body). Dad got suckered into signing up for a triathlon by some cute lady at the gym and did it for his 65th birthday to celebrate what he could physically do, and still does them. He just turned 80.
So OP... what do you want to be doing in 10yrs? Making another upsetting montage of your life, or adding some selfies of the duckling that turned into a swan?
