Rolling off a project that is hurting you is a blessing. I would focus on your health, and then look for a project that accommodates your needs.

My counselor, the engagement partner? The engagement director/SM is aware of this health issue and is pushing for my roll off (possibly the manager too)
My suggestion is to engage your counselor and CFL to make sure they are aware of the challenges you are having on this engagement before going to HR. You want to have an advocate just in case some funny business goes down that can impact your career. Also, it sounds like rolling of this project is not a bad idea. Good luck!
Agree with EY1. Is there a leader/mentor at your firm who you trust that you could have a conversation with first? It doesn’t have to be someone you currently work with - just someone that would be there for advice and to have your back. Definitely don’t do nothing!!! Your health and life comes first!!!
