There is no general rule. Each company is going to have a different policy about how long you have to come back for to not owe anything. 1 month? 6 months? Something else. Every employer and health/dental/life/who knows what else plan is going to have a different cost associated with it. And it may not just be the health insurance you’re responsible for but the salary as well. Ask your HR group or look for documentation on your company site.

You don’t in the US. Your benefits cover you at the point in time. There’s no clawback on your insurance, etc. like there could be for a signing bonus.
