Basically, you are looking for where your potential clients hang out. I assume when you say specialized home care, you mean for older or potentially disabled patients. I would reach out to physical rehabilitation centers and network with the leadership there. Essentially look at the “supply chain” or “business process” of getting old and see where the connections are. As morbid as it sounds you could also work at the end and talk to a hospice and understand how you can market best to these prospective clients.
As for gaining access in this age of limited access, LinkedIn is always a great place. I network like crazy over there and you would be surprised how open people are to jumping on a call, especially now with the lack of F2F engagement. Elder law attorney here. First, congratulations!! Second, here are some great potential referral sources: elder law attorneys, admission and discharge at hospitals, adult daycare companies, senior centers, financial advisors, CPAs, community chats (Facebook or otherwise), BNI or other networking groups, your local Chamber of Commerce, nursing homes (particularly their social workers who have to work on discharge plans). If your state has Medicaid waiver for in-home care then consider becoming a Medicaid waiver provider. It will probably pay you less, but it is likely to lead to a steady money drip.
Also, make sure you have really good insurance and an umbrella policy. Good luck and thank you for deciding to serve those who need care!

I'd recommend this book. Its a great practical workbook that takes you through all aspects of designing your offering, setting strategy, thinking about brand and differentiating etc. Its a really practical, explain, example, do yours work through and a great place to start.
If you're right at the beginning, it's not just thinking about how to find clients etc, it's how to engage and convince them that you're a great option even though you're an alternative to their current solution.... Then how are you going to secure, keep and grow more customers.
