If you think you have reached that point please do. Deffinately leave the firm you are working for. It will not affect your career, you had some time off sick (no disclosure necessary). Search for a new job long enough that you are comfortable that the culture is supportive and your work related anxiety will be eased
SA1, this kind of language is really damaging, even if not intended to be, and unless you're OP's therapist you shouldn't be armchair deciding the impact of employers and career choices on their mental health.
OP, it's absolutely possible for anxiety and depression to be unrelated to your work. A lot of us had both long before we got here. It's brain wiring. Does this job push some buttons that might be more sensitive with a mental health issue? Sure. But you can get treated, get well, and get back on your feet as a lawyer with a mental health management plan (medication, therapy, exercise, etc) when you're ready. You won't be the first.
The Georgia Lawyers Assistance Program hotline is 1-800-327-9631. It's confidential and 24/7, and can connect you with referrals, resources, and looks like you can get six counseling sessions per year too.
It sounds really serious, OP. Please take care of yourself. You are more important than your job. I would worry about the consequences later because you might cause yourself more anxiety by thinking about the impact. Whereas once you get into a better mental state, you might feel more optimistic and hopeful about the paths that are open to you. I think you will become a much stronger and better lawyer if you are healthy.

I’m sorry to hear you’re going through this too. I have never felt this way in my life, not to this extent.
The consequences (if there even are any) won’t matter if your mental health continues this way.
Contrary to popular opinion, there are great people in the practice of law who have also experienced what you are experiencing.
Get help. And if ever someone asks you about this time, you can say the honest thing: you saw what needed prioritizing and you used your excellent judgment to shift your focus to taking care of what needed taking care of.
Without your mental health, none of it matters. Address that first and foremost so that you can live a good life. Everything else will fall into place.
In order to live that life, you are wise to know you need assistance. Go get help and never look back ❤️
We had an associate in year 6 or 7 I think (can’t remember exactly) who checked himself into a place out west with no warning.. notified the partnership after he got there. He ended up taking a year off and the firm let him come back.. he had to make up the year, but still made partner after 8 years of practicing with the firm.. who knows how it will turn out with your firm, but I can tell you that none of the other associates judged him for it and as a partner he now mentors associates who come forward with issues.
I had to go to rehab (dual diagnosis of substance abuse and bipolar disorder). It put a lot of things in perspective and I'm a better, happier person now for it. There's no shame in asking for help and taking care of yourself.
I feel for you OP. Please get the help you need; that’s the most important. Everything else is secondary, and will work out in the end. If your employers are reasonable and supportive, they will help you figure out how to manage your career when you get back on your feet.

I don’t think you need to disclose why you are leaving if you don’t plan on returning to the firm.
And as far as other employment afterwards I would say you had a medical or family issue you needed to attend to.
