As a Canadian that worked in the states, I did it by winning a lot of awards. Also, there was (maybe still is ) a visa called the L1-A that's an inter office transfer, so it makes going from, for example, sid lee Toronto to Sid lee NYC, or even one holding co agency to another in the same holding co, much easier and faster. But it's for more senior staff. So, win a lot of awards or get a job at an agency that has an American office and then ask for a transfer after a while.
