Not unethical - just have to weigh the benefit of the days off against the payout you can get for unused days
Would be unethical to take a lot of “sick” days though. Some people will also judge you if you take two weeks off then put in your notice right when you come back
Make all your needed doctor appts and see them before leaving the firm
Get your CPE record and certificates as well.

Not unethical. I left a firm with unlimited PTO, so I took a ton of time off before I left since it wouldn’t be paid out.
No. You earned them. It may be underhanded to time your two weeks so that it coincides with a two week vacation- it would depend on if you have responsibilities to wrap up
