Could be lots of things. Network congestion, unit cabling (make sure you have at least cat 5e), building cabling (not a whole lot you can do then), or hardware related.
Are you testing over wireless? If so, plug in directly to your router and then test.
Also, test directly connected to your modem.

Now, I am getting 140 mbps on Ethernet, but only 50 mbps on WiFi. For some reason, it drops to 20 when I plug-in the TP-link extender (which claims to work up to 300 mbps).
It sounds like I may just need eero or any reasonable standalone router.

Currently using Netgear C3000.
Check the channel if using WiFi. Manually set the channel. Use wired and ONLY wired for anything speed intensive like gaming, 4K streaming, and working on VMs. Remove any performance based throttling on networking devices and disable any power saving features on your router.
Are you using modem router combo? I was using it and had the same speeds as you do for 100mbs internet. Got a separate router and turned the modem into bridge mode. Now my laptop on WiFi shows around 70-80 MBs vs 20 MBs when I was using modem router combo.

It sounds like I may just need eero or any reasonable standalone router. Hopefully, it will be sufficient to extend the signal into my office space two floors directly above the modem.
Your extender has to take your traffic and then send it to the main access point, so it will be slower. Eero does a better job, but there is a penalty for hoping over multiple devices.
