@M1, custom is a pre-built shirt which you do alterations. Bespoke is a shirt that is built from fabric for you.

OP, I have the answer for you - Regent Shop. They are a family owned business that has operated in Hong Kong for 75 years, and are situated in the shopping arcade of the Peninsula next to some extremely high end stores. The quality-price balance is excellent, with standard shirts costing approximately $140 shipped if you buy fewer than four shirts. If you buy four, you get a fifth free, which drops the average price to $112 or so - firmly into off the rack territory. This is for a full custom shirt made in any style you may desire, with a huge array of fabrics. While the standard fabrics are not unbelievably high quality like some of the top British brands (e.g. Hilditch and Key), they are pretty good and offer a wide array of styles, especially for the price. You can upgrade your fabric to something more premium if you so desire, and all of their shirts come with pearl buttons They visit pretty much every major US city twice a year. Alton Lane is great fit. I struggled finding anything that fit my body as well until them. Only issue is they require ironing if you travel.

Meanwhile, back on planet earth: if you’ve got unusual dimensions (football player’s neck and you need to wear a tie, extra-short arms, etc), I’d go with Proper Cloth.
If you’re in the normal range, Charles Tyrwhitt and TM Lewin two great London brands, just down Jermyn St from Turnbull & Asser. Each one has 4 different cuts, so they’re likely to have something that will fit you well
