I refuse to include that info on any forms even for doctors and dentist, never been refused service as a result, I think most people give it away without thinking about it but I do not see a need for a doctor/dentist to know that info
I always tell the doctor’s office that I don’t feel comfortable writing my SSN, and they always agree with me. As long as my health insurance is verified with the provider, I don’t get pressured to write my SSN in those forms. 👍🏼 #IdentityTheft

To be honest you’re 1000x more likely to get your identity stolen when it’s in a system. Generally you’re typical receptionist or md won’t be committing identity theft
Just refuse and pay cash
They ask because it’s helpful for insurance verification. It’s probably fine if you don’t since you likely have correct and active insurance coverage, but for patients who might not know their insurance coverage information or what other active coverage they have or give the wrong info, they can check using your SSN in case they can’t ever reach you again
