4 weeks before due date is std, and the first 5 calendar days are waiting period (if it falls over a weekend, you don't have to use PTO for all 5 days). This may end up being more or less than four weeks depending on when your baby actually arrives.
6 weeks of std starting after baby is born, followed by 6 weeks of paid parental time off (ppto) which you technically have the option of spreading out over six months if you choose.
If you work in a paid family leave state, and you choose to take it, it will be scheduled between std and the ppto.
FMLA provides UP TO 26 weeks of job protected leave starting the first day you are out (4 weeks before due date), and runs concurrently with all of the above. (So it's not necessarily 4/12/12 weeks as you state in your original post.)
