Depends on your company's policy, and possibly the state law. There are companies that demand a doctor's note for every instance of sick leave, as juvenile as it is.
I’ve heard of that. My work has never required a doctor’s note except for long absences (like if I miss a week due to illness). I’m in NY state, not sure if that changes anything
That's a little bizarre that they require a list of all visits...I haven't encountered that before. Are you using PTO for those non-FMLA appointments?
Sometimes I use PTO. Usually I have to use my vacation time. We’re given sick time for appointments but have strict rules on using it. It’s very annoying which is why I sometimes take these absences unpaid
