Ahh when this happens we do this:
- wash bum with water and soap at each diaper change
- pat dry with soft cloth / let it completely dry
- apply a THICK layer of Boudreaux’s Butt Paste or Triple Paste all around the bum / cover entire rash area to create a barrier / prevent contact with moisture
Rash should be gone in 1-2 days

Desitin and Boudreauxs butt paste are the best we have found. Let them be diaper free as much as you are able to. If you can, instead of wiping after a dirty, try to shower them off.
Also, on a go forward basis, apply aquaphor as a protective barrier to prevent future rashes. As newborns, we did this every diaper change and slowly dropped to just before bed.
Boudreaux’s butt paste worked for us too. And look up the babybum brush if you don’t already have one. Much better than having to smear it on with your fingers

Try triple paste next time, it worked better for us than Desitin!
Another vote for triple paste. Also you can use corn starch on the butt and then apply the triple paste. That worked wonders when my oldest had a really bad rash. Also try going without the diaper for a little while to air oht
For me Aquaphor baby works magic. Redness gone in several hours. And works well for all parts of the body.

Destin has been better in my case. I am sure you are doing this
- change to a different diaper brand if you can. For my son only honest worked
- up a size if you can and that might be lose
- I let my son bare bum for 15 mins after a feed as it usually takes 15 mins for him to pee after a feed and even if he does I lay him on a cloth diaper insert bare bum which was easily washable or throwable
Yes, leave him diaperless for as long as you can. If it doesn't clear up check with the ped it may be more than common diaper rash.

I did destin or triple paste and then put a layer of aquaphor over it. Agree on diaperless as much as is reasonably possible.
Same as others above - organic diapers (honest is what we ended up with but didn’t try other organics), water wipes/Honest wipes, butt paste, air out that booty! In the future, I’d also pay attention to frequency of diaper changes to prevent the rash. More frequent diaper changes seem to help prevent rashes overall. Good luck!!
We use aquaphor baby for the bums. Works the best for us and also easier to clean than Destin. For the red rashes, I put them in a baking soda soak for a few mins before bath time. It’s supposed to neutralize the ammonia irritation and it has worked like magic for us. I do it daily (which is usually not more than 2 days) until it gets better, air dry and generous layer of aquaphor baby.
I’d go with the Desitin if it’s that bad for now, until it’s healed and try to let him air out as much as possible. We used Pampers Swaddler diapers, best for ours with sensitive skin. I personally didn’t use diaper cream that much, the key we found was to make sure everything is as dry as possible before diapering. We used some plain cotton burp cloths as diaper cloths to always dry bum really well after wiping and before diaper. That kept baby clear 90% of the time for us and we’d use rash cream for a breakout. You’ll get the hang of what works best for your baby as you get to know each other. Hope you both feel better, you’re doing great!

100% agree on the air it out. We air my son out for a bit after removing his night time diaper and wiping him clean, after bath/before jammies, and any other time I can get away with it, lol. He has rarely had a rash and he has very sensitive skin, but when dad’s in charge he forgets to air and I can almost always tell it’s been a few days because he’ll get red. When he was less mobile we threw a towel down for him to lay in while he was airing, now that he can run around I strongly suggest a spot carpet cleaner. Then you don’t have to stress about mess because the clean is simple and effective if something does happen. We also size up in diapers at night to absorb extra and keep it off his skin (our kid pees more than any other kid I’ve seen at his size).
I have a stash of creams and ointments and find they work differently for different kinds of rashes. Burt’s Bees and Honest both have diaper creams and healing balm/ointment. Honest diaper cream during the day, Burt’s Bees diaper cream at night (lighter cream for day, heavier at night to keep the barrier but during the day you rub too much to get it off and irritate them more...), and then alternate during the day with the healing balm or ointment to actually heal the skin rather than just create a barrier. The healing balm and ointment are great for other skin issues like drool rash on the cheeks, chin, and neck, etc. in my experience every kid is a bit different. Loved butt paste when nannying but for my own I’ve found I prefer the above options. If it doesn’t feel like it’s working, switch it up to find the right combo. Generally, if it’s been 2 hours between changes (and they’re not clearly soaked through and just aggravated by being wet with urine for a while) and it still looks raw or you see no improvement whatsoever, rotate to the next cream option.

Best one we ever tried was a mix
First lather the actual red burn part in A and D ointment.
Then cover that area and more...with the purple Desitin....lather it thick “like cake frosting”
The AD helps heal and the desitin keeps it all sealed off from more irritation

Just a heads up from an allergy mom…..if you find a “bullseye” it could also be a food allergy (from your milk if you are BF or from the formula). My kids had that with egg. I know other moms had that with dairy.

Yep, bad diaper rash warrants consideration of allergies for sure.
If it starts to get bumpy, that means you’re dealing with yeast and need lotrimin. If really bad then call your ped to get nystatin.
I would rinse my daughter's butt with camomile tea. +air it out, +red Bordeaux paste. This was the fastest I found it would go away

Kindly, Aquaphor is not a moisturizer. It’s an occlusive - it will do a great job of holding moisture in but it does not moisturize in and of itself. Definitely recommend hyaluronic acid, facial mists and moisturizing lotions if your skin needs hydration 👌
