My nanny doesn’t have children. Our nanny has been with my twins since they were 4 months old (they are almost 6 and she is 70 now) and we very much treat her as part of the family. So we get her stuff from us for anything I would do for a family member. She does the same for my girls - I never expect anything from her for my husband or me. We also do some craft from the girls for her for Christmas and her name day (she is Greek so she doesn’t celebrate her birthday, but her name day). If she had kids, I’d say if the kids come around my kids then I’d give to them too, because they would be part of the family as well.
Just some sweets. I was a nanny for an extremely wealthy family (they lived in a 5million dollar home), and they always went above and beyond on my birthday & Christmas. I did not have children back then, but even if I did, I wouldn’t have expected any presents from my boss to my kids. Maybe you can give them a nice box of cupcakes and a bunch of balloons? Make it super cute :) but I wouldn’t do more than that.
I have a very modest income but we give out after school tutor (who used to supervise virtual school all day) a gift and we get cupcakes (bc of pandemic) and sing happy birthday. I think a gift ($50-100 range) and cupcakes is appropriate. Gift for Christmas if she celebrates as well. If her kids are there with you maybe cupcakes and singing on the same day for all 3. I think a small gift ($20 range) for each child is appropriate if they spend time at your home.

I gave her and her daughter gift cards for their bdays and Xmas - amount would vary on your income.

That is super sweet of you to give your nanny’s kids presents for their birthdays. I would say that is not necessary but agree with something smaller if you do. I like books for kids gifts because you can never go wrong.
Two nice books for each child - wrapped with a ribbon is a nice idea. Or a book with a game like UNO.
