Also, feels kind of bogus to call them out on something on a form for their boss's eyes when I would never bring it up to them directly.

I mean, the question is what is the point of the review? Is it a forward looking continuous feedback program? If so, then you should talk to the person and note why you wrote what you wrote. As long as it’s constructive, it should be well received.
If the point is to give people ratings and rank them, then yea leave the petty BS out of it.
Really think about. No one is perfect.
Can I just say something like "I have not identified any weaknesses" every time or is that a cop out?
Imo this is an appropriate answer if you truly don’t see anything. This question is helpful if there’s recurring issues and wanting to formally document it if bringing it up to them hasn’t proven to be effective.
