Admission nurse and case manager is usually the same thing different companies may change the name or if they have so may SOC (start of cares) they could hire an RN to complete OASIS (Medicare Admission check list basically) only. However case managers will also provide nursing care. Dressing changes, access central line, central line dressing chgs, administer Iv meditation, stage wounds & take photos, monitor progress and communicate with pcp and other care team members for patient care. You ensure the patient has access to dme and meds, and that they are taking care of themselves. The goal of homehealth is to empower the patient and or family, but it doesn't always work out that way. For either position be prepared for paperwork (or tablet work) lots of boxes to check.
My guess would be they are trying to hire an RN for OASIS admissions for SOC only.
Well a home health nurse case manager supports the ongoing needs of a patient in their home. Managing services and possible increases or decreases of services through assessment. And you do this for multiple patients in a defined geographical location and have a caseload.
Make your own schedule, chronic disease management, TONS of unnecessary charting (thank you MCR). Best part is the autonomy. I’m PRN home health but our FT nurses work on productivity points. Great to have one on one patient care. Totally different aspect of nursing. You either love it or hate it is my experience.
