I've worked at the same engineering firm for 4 years. I'm currently making $161k with only a $750 bonus, but excellent vacation (4 weeks, cash out anytime, rolls over indefinitely), 45 hours a week. I have an interview with a recruiter at Guidehouse this week for a Technical Project Manager role. It seems to be focused in the government space and requires a security clearance. What sort of salary and benefits could I expect for this sort of role at Guidehouse?

What kind of problems? Like blood clots or like trouble sleeping. I think for the former you should definitely talk to your project team. For sleep and energy I've always found working out to work. Ask, check out IV/hydration therapy, I e not tried it but one of their selling points is travel.

Just don't book red eye flights on Thursday. Book an 8 am flight on Monday..get there when you get there. Book a flight for 1-2ish on Thursday so that you get home at a decent hour (before midnight). If they have a problem with that, tell them to let you come in on Sunday's or you need to roll off to find something with more balance
I did West coast to East coast for almost a year. Talk to your team about a flexible travel schedule - for example, I would wake up early on Mondays and work from home until noon or so (getting in most of a work day in East coast time) and then get on an afternoon flight and get to the client city around midnight. A Thursday flight around 5 pm gets you home around 8. See if you can pull off a remote week (or two) per month. My team was reasonable and realized it was better for all of us (client included) if I was working from home a bit more and therefore more rested given the draining nature of that travel.
Did what A5 did for my last west to east project. It is better for everyone to have you rested and functioning.

One month and health issues would suggest something slightly more chronic than the last month.
If it is a time zone / sleep deprivation issue then you're hitting the time when you body starts getting a little less dependent upon the "local" rhythm to rest.....

@OP you might be able to work something out with your hotel to check in early (~8am) and nap for a couple hours. I have a colleague currently doing that. Otherwise, definitely exercise on Sundays and it will help you sleep loads better on the plane. Also worth investing in some noise canceling headphones and a neck pillow.
