Heard about a partner at monitor that had $10k of receipts he handed to an admin 3 months late.
A guy got fired for driving his car to the airport but expensing in fake cab rides every week
Two senior managers we're so far behind on reporting their expenses that it required the project controllers to do it themselves. I don't know how they did it but they found out that those two senior managers had not expensed over two months of bills and this screwed up the entire financial projections for three separate work streams on the project. We actually had to send three consultants to the bench because of these two idiots because they were too lazy to go online and import their AMEX bill and spend the 15 minutes necessary a week to do it. Somehow they kept their jobs.

And a long time ago I heard about three guys being fired to somehow embezzling personal cellphone expenses.
They check credit card statement and see no cabs

I had American Express call and threaten to cut off my card due to late payments. I was 6 months behind in expenses.

@PwC: yea. I take uber on my personal card and manually import it sometimes.
BCG buddies loved "cabitrage" - take the train home but expense a cab using the handful of cab receipts they give you.
